Report Barriers
All levels of employees must work together to improve accessibility in all areas and to establish a culture of inclusion.
It is important to report barriers that you may experience as a person receiving services, as a team or as a family member, so that we can address the barriers, try to remove them in a timely manner or advocate for change.
~ This form will go to the Executive Director. They will respond as soon as possible. ~
